Manager responsible for day-to-day operation and support of its
Capital Campaign to raise $3.5 million to build a hospice care center
in Brunswick County. The position requires an energetic, highly
organized, self-managing, self-starter with confidence and personal
presentation needed to sustain relationships with community leaders
and donors. Undergraduate degree in business administration, marketing,
or other related field or equivalent required. CFRE a plus. Demonstrated

Outreach Provider Representative,
Brunswick - 1.0
FTE Works under the supervision of the Director of Outreach. Develops
well-informed, responsive relationships with physicians and their
staff, hospital staffs, long-term care facilities, other health
care providers, through communication and education to increase
patient census and length of stay. Acts as a resource to physicians,
patient/families, and assigned facility staff regarding end-of-life
issues and palliative care. Acts as a consultant to assess patients
for appropriateness of admission when needed. Supports the development
and execution of LCFH&LCC’s outreach goals and plan. To
visit physician offices, hospitals and extended care facilities
on a consistent basis to build awareness of access to LCFH&LCC
programs and create a visual presence in the community. Degree in

professional grief counseling and education to families of former
patients. Group grief education and support is provided to families

services. The Bereavement Counselor is responsible for continuing
