Weekday On-Call Nurse – Onslow/Pender counties (Jacksonville/Burgaw, NC)

1.0 FTE

The On-Call Nurse provides direct patient care for patients with life limiting illnesses.  The patient population can include newborns through geriatric patients.  The On-Call Nurse is responsible for using the Nursing Process to effectively manage the Plan of Care for each patient based on appropriate nursing standards, policies and procedures.  Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes.  The On-Call Nurse is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing.  The nurse performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program. LPN requirements: Education:  Graduate of an approved school of practical nursing. Current license to practice professional nursing in the State of North Carolina. Two years’ nursing experience.  Prefer Home Care, Hospice or related oncology experience.

LPN Triage (Weekends) – New Hanover County (Wilmington, NC)

0.6 FTE

The Licensed Practical Nurse provides direct patient care under the supervision of the Professional Registered Nurse, for patients in life limiting illnesses, based on appropriate nursing standards, policies and procedures. The patient population can include newborns through geriatric patients. The Licensed Practical Nurse participates in assessments/data collection and treatments, as delegated by the RN, based on a defined Plan of Care. Performs other duties as assigned. Graduate of an approved school of practical nursing. Current license as a practical nurse in the State of North Carolina. One-year experience required. Home care, hospice or oncology experience preferred.

Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina. Minimum of two years’ experience and at least one year in home care or by other individuals under general supervision of a Registered Nurse. Previous HA experience in a hospice, home health, or oncology preferred.

Clinical Nurse Manager – Care Center – Brunswick County (Bolivia, NC)

1.0 FTE

The Clinical Nurse Manager assists with management of daily patient care activities and clinical operations. Is accountable for assuring quality outcomes and appropriate resource consumption.  Works closely with the Director of Patient care services to achieve positive financial outcomes for the geographical area. Direct oversight and supervision of the hospice aide.  The patient population can include newborns through geriatric patients.  Directs all activities in a highly professional manner, demonstrates behavior that exemplifies the LCFH mission statement, and applies principles of leadership and professional standards to all job responsibilities.  Performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program. BSN or master’s degree in nursing or related field preferred. Current license to practice professional nursing in the State of North Carolina / South Carolina. Minimum of three years nursing experience in an acute care setting is required. A minimum of three years management experience is required, preferably in an acute care, home care or residential setting. Hospice experience is preferred.

Social Worker – PRN – All Locations

.10 FTE

The Hospice Social Worker provides professional social work services directly to terminally ill patients and their families. As a member of the IDT, the Social Worker provides consultation with team members about psychosocial care issues and coordinates social work services with the team. The Social Worker is responsible for continuing his/her own professional development to enhance practice. The Medical Director provides oversight of this position, via the IDT meetings. Performs other duties as assigned. MSW preferred, minimum of BSW, from a CSWE accredited School of Social Work. NCLCSW recommended, not required for NC. LMSW for SC required. At least two years of professional social work experience, preferably in a health care setting.
