Hospice Aide -Night Shift – New Hanover County (Wilmington, NC)

0.9 FTE

The Hospice Aide works under the supervision of the Professional Registered Nurse to provide a variety of basic patient care activities to meet personal needs and comfort to patients. Care is based on Hospice Aide’s level of registration and agency policies and procedures. The patient population can include newborns through geriatric patients. The Hospice Aide assists in clerical and stocking duties for the department or agency. Performs other duties as assigned. High school diploma or equivalent. Completion of Hospice Aide course. Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12 month basis. Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation. Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina. Minimum of two years’ experience and at least one year in home care or by other individuals under general supervision of a Registered Nurse. Previous HA experience in a hospice, home health, or oncology preferred.

Patient Scheduler – Home Office – New Hanover County (Wilmington, NC)

1.0 FTE

The Patient Scheduler works under the supervision of the Patient Scheduler Manager. The primary function of this position is coordinating the delivery of Centralized Scheduling services for all patients receiving care from Lower Cape Fear Hospice staff. The Patient Scheduler will be responsible for entering discipline specific scheduled visits for all patients; to include the RN Case Managers, LPNs, Social Workers, Certified Nursing Assistants, Chaplains, and other designated staff as deemed necessary. The Patient Scheduler must associate and assign employees to patients for scheduling purposes. Responsible for communication on a high level with all staff and patients involving patient care services. Responsible for the delivery of scheduling services by assigning and prioritizing tasks. Accountable for quantity and quality of work in the Central Scheduling Department. Performs all Central Scheduling functions. The Patient Scheduler will possess a good working knowledge of the Allscripts Product. Provide backup coverage as needed in the Resource Center. Required to perform all duties in a highly professional manner, demonstrate behavior that exemplifies Lower Cape Fear Hospice’s mission and values. Performs other appropriate duties as assigned. High school diploma or an equivalent. Associate’s degree or equivalent preferred, but not required. Minimum of one year of phone skills, sales and/or customer service background.

Clinical Nurse Manager – Care Center – Brunswick County (Bolivia, NC)

1.0 FTE

The Clinical Nurse Manager assists with management of daily patient care activities and clinical operations. Is accountable for assuring quality outcomes and appropriate resource consumption.  Works closely with the Director of Patient care services to achieve positive financial outcomes for the geographical area. Direct oversight and supervision of the hospice aide.  The patient population can include newborns through geriatric patients.  Directs all activities in a highly professional manner, demonstrates behavior that exemplifies the LCFH mission statement, and applies principles of leadership and professional standards to all job responsibilities.  Performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program. BSN or master’s degree in nursing or related field preferred. Current license to practice professional nursing in the State of North Carolina / South Carolina. Minimum of three years nursing experience in an acute care setting is required. A minimum of three years management experience is required, preferably in an acute care, home care or residential setting. Hospice experience is preferred.

Social Worker – PRN – All Locations

.10 FTE

The Hospice Social Worker provides professional social work services directly to terminally ill patients and their families. As a member of the IDT, the Social Worker provides consultation with team members about psychosocial care issues and coordinates social work services with the team. The Social Worker is responsible for continuing his/her own professional development to enhance practice. The Medical Director provides oversight of this position, via the IDT meetings. Performs other duties as assigned. MSW preferred, minimum of BSW, from a CSWE accredited School of Social Work. NCLCSW recommended, not required for NC. LMSW for SC required. At least two years of professional social work experience, preferably in a health care setting.

Controller – Home Office – New Hanover County (Wilmington, NC)

1.0 FTE

The Controller is responsible for managing the daily technical functions of the accounting division including accounts payable, cash receipting, miscellaneous receivables, payroll, management of general ledger, and statistical/financial reporting. This position carries out assigned administrative tasks as specific to the functional operation of the corporate office. Supervises and coordinates activities of supporting staff. Maintains a close interpersonal working relationship with all agency staff. Performs other duties as requested. Degree in Business Administration with a concentration in Accountancy from a four-year college or university. CPA preferred. Three years of related experience to include supervisory experience.

RN Float – New Hanover County (Wilmington, NC)

1.0 FTE

The Professional Registered Nurse provides direct patient care for patients in life limiting illnesses. The patient population can include newborns through geriatric patients. Is responsible for using the Nursing Process to effectively manage the Plan of Care for each patient based on appropriate nursing standards, policies and procedures. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. The Professional Registered Nurse is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program. Current license to practice professional nursing in the State of North Carolina. Two years’ experience. Prefer Home Care or Hospice or related oncology experience.
