Outreach Provider Representative  – Brunswick County (Bolivia, NC)

1.0 FTE

Works under the supervision of the Director of Resource Center. Representative will develop well-informed, responsive relationships with physicians and their staff, hospital staffs, long-term care facilities, and other health care providers, through communication and education to increase patient census and length of stay. Representative will act as a resource to physicians, patient/families, and assigned facility staff, regarding end-of-life issues and palliative care. Also acts as a consultant to discuss eligibility for agency programs and services. Supports the development and execution of LCFH outreach goals and plan. Representative will visit Physician’s offices, hospitals and extended care facilities on a consistent basis to build awareness of access to LCFH programs and create a visual presence in the provider community and the community at large. Degree in business, marketing or related field preferred. Valid North Carolina Driver License required. Three years’ experience: preference in hospice or home care or experience in marketing, public affairs, community relations or hospital liaison nurse.

Development Coordinator – Home Office – New Hanover County (Wilmington, NC)

1.0 FTE

Provides coordination and logistics of fundraising, memorial, community events, and donor recognition activities. Interacts with a wide range of diverse publics to include patients, their family members, community leaders, volunteers, donors and staff.  Performs administrative duties of a confidential manner. The Development Coordinator engages in planning, conceptualizing, implementing and assisting with full details of special events for the Development office. The fundamental goal is to increase community awareness, advocacy and financial support to the organization. Will also be responsible for access to or knowledge of protocol, donor and friend information, and donor database. This position requires a flexible schedule. Weekend and night work is required throughout the year. Bachelor’s degree in marketing, communications, hospitality management or related field or related work experience or an equivalent combination of education and experience. Event coordination experience required. CFRE a plus. Two years’ related work experience or an equivalent combination of education and experience. Fundraising experience a plus.

Administrative Assistant to Senior Management – Home Office – New Hanover County (Wilmington, NC)

1.0 FTE

Works under the supervision of Vice President of Development, and also supports Director of Finance and Director of Human Resources. Primarily responsibility is to assist senior management in performing a diverse array of administrative duties and a wide range of responsibilities using discretion, independent judgement and senior level administrative support skills. This position requires professional poise and friendly people skills; a high degree of organization, an individual who works well in team environments, possesses advanced written and verbal communication skills, demonstrates advanced skills in current computer technology, and has a strong ability to prioritize and multi-task. Performs other duties as assigned. High School Diploma required. Bachelor’s Degree preferred or applicable experience in an administrative assistant role. Five years’ experience as an administrative assistant or secretary to senior level individual(s) (healthcare experience preferred). Experience should include interaction with all levels within an organization, including board members. Skills to create correspondences and ability to prioritize tasks. Demonstrated writing and editing.

RN Case Manager – New Hanover County (Wilmington, NC)

1.0 FTE

The Professional Registered Nurse provides direct patient care for patients in life limiting illnesses. The patient population can include newborns through geriatric patients. Is responsible for using the Nursing Process to effectively manage the Plan of Care for each patient based on appropriate nursing standards, policies and procedures. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. The Professional Registered Nurse is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program. Current license to practice professional nursing in the State of North Carolina. Two years’ experience. Prefer Home Care or Hospice or related oncology experience.

Nurse Practitioner Weekends – Home Office/NHRMC – New Hanover County (Wilmington, NC)

1.0 FTE

The Nurse Practitioner provides direct patient care for patients with life limiting illnesses.  The patient population will reflect that specified by the nurse practitioner’s master’s degree. Is responsible for using expert skills in history taking, physical examination, initiation and interpretation of tests, analysis of data, and the implementation and evaluation of plans of care. Effectively manages the Plan of Care for each patient based on clinical knowledge, data, patient preference and benefits to the patient.  Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. Works with Palliative Care Center physicians in the hospital, nursing homes and physician’s office. The Nurse Practitioner is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing.  Performs other duties as assigned. Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program. Graduate of a Master’s degree in nursing program with a major or post masters as a nurse practitioner. Current license to practice professional nursing in the State of North Carolina / South Carolina. Approval for practice as a nurse practitioner by the North Carolina Board of Nursing and the North Carolina Board of Medical Examiners.Minimum of five years general medical experience in inpatient, outpatient and nursing home settings. Two years as a nurse practitioner preferred. Experience in an acute care setting desirable. Demonstrated competence in leadership and teaching. Demonstrated achievement and familiarity with administrative nursing techniques.

Chaplain – Bladen County (Whiteville, NC)

.15 FTE

The Hospice Chaplain provides spiritual care services directly to terminally ill patients and their families in a clinical setting, and indirectly by way of linkage with local clergy and religious communities. As a member of the IDT, the Chaplain provides consultation with team members about spiritual care issues, and coordinates chaplain services with the team. The Chaplain is also available to provide community education and consultation, regarding spiritual care and end of life issues and concerns, when requested. The Chaplain is responsible for his/her ongoing continuing education, to assure that skills and knowledge are enhanced.Minimum of Bachelor’s degree in theology or divinity, and at least one unit of Clinical Pastoral Education or its equivalent. Master’s degree preferred. Counseling education is desirable. Professional license/certification preferred, but not required upon employment. At least 2 years of experience in pastoral care, counseling, or the ministry,preferably in a health care setting.

Human Resources Representative Customer Service – Home Office – New Hanover County (Wilmington, NC)

Works under the supervision of the Director of Human Resources. Performs duties of a wide array of both confidential and highly visible nature to assist in administrative support, and coordinating activities relating to Human Resources Operations. This includes assisting in the application of agency personnel policies, procedure and practices, maintenance of human resource documentation and employee personnel files, and application of federal and state law and regulation. This position facilitates the HR department’s efforts regarding Reward and Recognition and wellness.  This position assists in the recruitment, retention, new employee orientation, employee communication and recognition, employee relations, aspect of training, and the maintenance of HRIS, payroll and other applicable HR applications. Balances multiple tasks and duties and works under deadlines. Performs other duties as assigned. Associate’s degree (AA) or Certificate or equivalent from a one-year college or technical school, with emphasis preferred in business administration, human resources management or related field preferred. Two years HR experience in a healthcare setting preferred.

Hospice Aide PRN – Horry County (Myrtle Beach, SC)

.10 FTE

The Hospice Aide works under the supervision of the Professional Registered Nurse to provide a variety of basic patient care activities to meet personal needs and comfort to patients. Care is based on Hospice Aide’s level of registration and agency policies and procedures. The patient population can include newborns through geriatric patients. The Hospice Aide assists in clerical and stocking duties for the department or agency.  Performs other duties as assigned. High school diploma or equivalent. Completion of Hospice Aide course. Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12 month basis. Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation. Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of South Carolina. Minimum of two years’ experience and at least one year in home care or by other individuals under general supervision of a Registered Nurse. Previous HA experience in a hospice, home health, or oncology preferred.

The Hospice/Palliative Care Physician is responsible for the medical component of the hospice and palliative care program. Acts in the capacity of hospice medical director overseeing the medical components of care to hospice patients in all settings and acting as attending physician for patients in acute care hospice facilities. Performs consultative services to patients and families with life limiting illnesses; helps to determine and implement policy concerning the medical component of the hospice and palliative care program and services; supervises direct medical care of midlevel providers. Acts as a resource to the medical community and community at large with regard to end-of-life issues and provides education regarding pain and symptom management. Graduate of an accredited school of medicine or osteopathy with a degree in medicine or osteopathy. Current North Carolina/ South Carolina practicing physician licensure, valid driver license; credentialed member of hospital staff. Has completed an accredited residency program and has at least five years of experience in end-of-life care. Board Certified in Hospice and Palliative Medicine preferred.
