Outreach Provider Representative – New Hanover County (Wilmington, NC)

1.0 FTE

Works under the supervision of the Director of Patient Access. Representative will develop well-informed, responsive relationships with physicians and their staff, hospital staffs, long-term care facilities, and other health care providers, through communication and education to increase patient census and length of stay. Representative will act as a resource to physicians, patient/families, and assigned facility staff, regarding end-of-life issues and palliative care. Also acts as a consultant to discuss eligibility for agency programs and services. Supports the development and execution of LCFH outreach goals and plan. Representative will visit Physician’s offices, hospitals and extended care facilities on a consistent basis to build awareness of access to LCFH programs and create a visual presence in the provider community and the community at large. Degree in business, marketing or related field preferred. Valid North Carolina Driver License required. Three years’ experience: preference in hospice or home care or experience in marketing, public affairs, community relations or hospital liaison nurse.

Administrative Coordinator – Brunswick County (Bolivia, NC)

1.0 FTE

The Administrative Coordinator for BCC will assist the Clinical Coordinator in coordinating the interdisciplinary operations of the Center. Serves as key liaison for referral sources, vendors and contracted services. Ensures adequate supply of medical and other supplies while monitoring costs and charges for appropriateness. Ensures medical records meet agency and regulatory requirements. Maintains 24/7 availability for emergency building and staff issues. Performs other duties as assigned. High school diploma or an equivalent. Associate’s degree or equivalent preferred. Minimum of one year medical record, medical office or hospice experience required.

Hospice Aide – New Hanover County (Wilmington, NC)

1.0 FTE

The Hospice Aide works under the supervision of the Professional Registered Nurse to provide a variety of basic patient care activities to meet personal needs and comfort to patients. Care is based on Hospice Aide’s level of registration and agency policies and procedures. The patient population can include newborns through geriatric patients. The Hospice Aide assists in clerical and stocking duties for the department or agency. Performs other duties as assigned. High school diploma or equivalent. Completion of Hospice Aide course. Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12 month basis. Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation. Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina. Minimum of two years’ experience and at least one year in home care or by other individuals under general supervision of a Registered Nurse. Previous HA experience in a hospice, home health, or oncology preferred.

Development Manager – Brunswick County (Bolivia, NC)

1.0 FTE

The Development Manager is responsible for meeting or exceeding fundraising goals for the achievement of major revenue growth through special events and major gifts. Plan special fundraising events; coordinate and oversee logistics; manage budget and financial records; evaluate cost-effectiveness, profitability, and public awareness of events; compile and analyze event statistics; and create event plan for the following year. Recruit senior volunteer leaders and manage volunteer committees to assist in reaching organizational fundraising goals. This role requires creativity, strategic smarts, and strong organizational skills and has the opportunity to create unparalleled, authentic events.Bachelor’s degree in marketing, communications or related field and 3 years’ related work experience or an equivalent combination of education and experience. Event coordination experience, Major gifts fundraising or sales experience required. CFRE a plus. Three years’ related work experience or an equivalent combination of education and experience. Fundraising experience required. Demonstrated successful experience soliciting and closing sales; building and maintaining long-term relationships with constituents. Successful experience Customer Relations Management. Demonstrated knowledge of current trends in charitable giving, particularly in the areas of events planning, and securing sponsorship.

RN Case Manager – New Hanover County (Wilmington, NC)

1.0 FTE

The Professional Registered Nurse provides direct patient care for patients in life limiting illnesses. The patient population can include newborns through geriatric patients. Is responsible for using the Nursing Process to effectively manage the Plan of Care for each patient based on appropriate nursing standards, policies and procedures. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. The Professional Registered Nurse is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program. Current license to practice professional nursing in the State of North Carolina. Two years’ experience. Prefer Home Care or Hospice or related oncology experience.

The Clinical Coordinator assists with management of daily patient care activities and clinical operations.  Is accountable for assuring quality outcomes and appropriate resource consumption.  Serves as resource to health care/clinical staff community resources within service area.  The patient population can include newborns through geriatric patients.  Directs all activities in a highly professional manner, demonstrates behavior that exemplifies the LCFH mission statement, and applies principles of leadership and professional standards to all job responsibilities.  Performs other duties as assigned. Graduate of an approved school of nursing, either through an AD, Diploma or BSN program.  BSN or master’s degree in nursing or related field preferred. Current license to practice professional nursing in the State of North Carolina / South Carolina. Minimum of three years nursing experience in an acute care setting is required.  A minimum of three years management experience is required, preferably in an acute care, home care or residential setting.  Hospice experience is preferred

RN Clinical Liaison – Home Office/NHRMC  (Wilmington, NC)

1.0 FTE

Works under the supervision of the Director of the Resource Center to coordinate admissions and discharges within the hospitals, long term care facilities and assisted living facilities. Coordinates the in-patient care for Palliative and Hospice patients. Develops well-informed, responsive relationships with physicians and their staff as well as other health care providers in the hospital, long term care facility and assisted living settings through communication and education to increase census and length of stay.  Acts as a resource to physicians, patient/families, and other facility staff regarding end-of-life issues and palliative care. Acts as a referral source/consultant to assess patients for appropriateness of admission and begins process. Supports the development and execution of LCFH& LCC’s marketing communications goals and plan. The Professional Registered Nurse is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Works with Palliative Care Nurse Practitioners and Physicians in the hospital setting. Performs other duties as assigned. Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program.  Degree in business, marketing or related field preferred. Current license to practice professional nursing in the State of North Carolina. Two years’ experience. Prefer hospice or home care.  Prefer experience in marketing, public affairs, community relations or hospital liaison nurse.

Human Resources Representative – Customer Service – Home Office – New Hanover County (Wilmington, NC)

Part-time (20-25 hours/week) with opportunity for full-time in future

Works under the supervision of the Director of Human Resources. Performs duties of a wide array of both confidential and highly visible nature to assist in administrative support, and coordinating activities relating to Human Resources Operations. This includes assisting in the application of agency personnel policies, procedure and practices, maintenance of human resource documentation and employee personnel files, and application of federal and state law and regulation. This position facilitates the HR department’s efforts regarding Reward and Recognition and wellness.  This position assists in the recruitment, retention, new employee orientation, employee communication and recognition, employee relations, aspect of training, and the maintenance of HRIS, payroll and other applicable HR applications. Balances multiple tasks and duties and works under deadlines. Performs other duties as assigned. Associate’s degree (AA) or Certificate or equivalent from a one-year college or technical school, with emphasis preferred in business administration, human resources management or related field preferred. Two years HR experience in a healthcare setting preferred.

The Information Technology Specialist analyzes, configures, installs and maintains desktop computers, laptops, phones, printers, copiers, peripherals and software in support of the agency’s functions. Responsible for managing the Help Desk queue and responding to Help Desk requests from end-users. Performs other duties as assigned. BS in Computer Science or related technical degree preferred, while an Associates degree with three years of customer support experience is acceptable. Three years of customer support in Information Technology.

Hospice/Palliative Care Physician – Columbus/Bladen counties (Whiteville/Elizabethtown, NC)

1.0 FTE

The Hospice/Palliative Care Physician is responsible for the medical component of the hospice and palliative care program. Acts in the capacity of hospice medical director overseeing the medical components of care to hospice patients in all settings and acting as attending physician for patients in acute care hospice facilities. Performs consultative services to patients and families with life limiting illnesses; helps to determine and implement policy concerning the medical component of the hospice and palliative care program and services; supervises direct medical care of midlevel providers. Acts as a resource to the medical community and community at large with regard to end-of-life issues and provides education regarding pain and symptom management. Graduate of an accredited school of medicine or osteopathy with a degree in medicine or osteopathy. Current North Carolina/ South Carolina practicing physician licensure, valid driver license; credentialed member of hospital staff. Has completed an accredited residency program and has at least five years of experience in end-of-life care. Board Certified in Hospice and Palliative Medicine preferred.
