Physician, Palliative Care/Hospice - 1.0 FTE The Palliative Care/Hospice Physician is responsible for the medical component of the palliate care program, performs consultative services to patients/residents and families with life limiting illnesses; helps to determine and implement policy concerning the medical component of palliative care program and services; and supervises direct medical care with other members of the palliative care team. Acts in the capacity of hospice medical director overseeing the medical components of care to hospice patients in all settings.Performs other duties as assigned. Graduate of an accredited school of medicine or osteopathy with a degree in medicine or osteopathy required. Current NC practicing physician licensure required. Has completed an accredited residency program and has at least five years of experience in end-of-life care, EPEC trained preferred. Maintains a board certification by ABHPM or will sit for the first available board examination for Hospice and Palliative Medicine administered within six months of hire. Nurse Practitioner - 1.0 FTE The Nurse Practitioner provides direct patient care for patients with life limiting illnesses. The patient population can include adults (45-64 years) through geriatric patients (65+ years). Is responsible for using expert skills in history taking, physical examination, initiation and interpretation of tests, analysis of data, and the implementation and evaluation of plans of care. Effectively manages the Plan of Care for each patient based on clinical knowledge, data, patient preference and benefits to the patient. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. Works with Palliative Care Center physicians in the hospital, nursing homes and physician’s office and patient’s homes. The Nurse Practitioner is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program required. Graduate of a master’s degree in nursing program with a major or post-masters as a nurse practitioner required. Current license to practice professional nursing in the State of North Carolina. Approval for practice as a nurse practitioner by the North Carolina Board of Nursing and the North Carolina Board of Medical Examiners required. Minimum of five years general medical experience in inpatient, outpatient and nursing home settings required. Minimum of two years to three years in geriatric nursing with two years as a nurse practitioner required. Experience in an acute care setting desirable. Demonstrated competence in leadership and teaching. Demonstrated achievement and familiarity with administrative nursing techniques. CNA – Casual Pool, Whiteville Care Center - 0.1 FTE The Nursing Assistant works under the supervision of the Professional Registered Nurse to provide a variety of basic patient care activities to meet personal needs and comfort to patients. Care is based on Nursing Assistant’s level of registration and agency policies and procedures. The patient population can include newborns through geriatric patients. The Nursing Assistant assists in clerical and stocking duties for the department or agency. Performs other duties as assigned. High school diploma or equivalent required. Completion of Nursing Assistant course required. Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina required. No prior experience is required. Previous NA experience in a hospice, home health, or oncology preferred. Clinical Liaison-Hospice/Palliative Care, Corporate - 1.0 FTE Coordinate admissions and discharges within the hospitals. Coordinates the in-patient care for Hospice patients. Performs initial review of charts on Palliative Care referrals and initial visit to patient and family in hospital. Develops well-informed, responsive relationships with physicians and their staff as well as other health care providers in the hospital settings through communication and education to increase census and length of stay. Acts as a resource to physicians, patient/families, and other hospital staff regarding end-of-life issues and palliative care. Acts as a referral source to assess patients for appropriateness of admission and begins process. Supports the development and execution of LCFH&LCC’s marketing communications goals and plan. Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program required. Degree in business, marketing or related field preferred. Current license to practice professional nursing in the State of North Carolina required. Minimum of two years nursing and three years sales experience preferred.
